Our mission is to provide services, resources and support to reduce homelessness, addiction and human trafficking that is victimizing our young. Our partnerships with other community organizations helps us redirect our young toward a better lifestyle where attaining their goals and dreams is possible

Who are we?

Lived Experience Professionals

Another Chance 4 Hope (AC4H) is a nonprofit organization, founded in March 2020, by lived-experienced professionals committed to promoting health and wellness within underserved BIPOC communities. Originally focused on serving unsheltered youth, we expanded our reach due to the growing needs in our community and now work with all BIPOC residents. What began in 2017 with home-cooked meals has grown into a trusted community resource and support system for youth and families.

We connect young people directly to vital services and resources, breaking down barriers through advocacy, mentorship, and strong community partnerships. Our work focuses on substance use and opioid prevention, harm reduction, human trafficking prevention, and access to basic needs like food, clothing, and hygiene.

Through partnerships with landlords, educators, and community agencies, we help youth secure safe housing, build life and relationship skills, and pursue education and personal growth. At AC4H, we believe every young person deserves to be seen, supported, and given another chance for hope.

Leondray Owens Co Founder/CEO He has dedicated his life to working with youth, using his personal experiences as a powerful example to help guide them away from poor decisions. His journey began in Memphis, expanded to Chicago, and now impacts the youth of the Twin Cities area. With his authentic humor and gentle spirit, he effortlessly connects with others, bringing laughter and creating a positive, welcoming environment.
Angela Cuellar Co Founder/Executive Director A passionate leader with lived experience, she has dedicated her career to working with youth since 2009. As a Certified Peer Recovery Specialist and Certified Suicide Prevention Specialist, she brings invaluable expertise to her work. An alumni of the Nexus Board and Commission Leadership Institute and with certified training from Streetworks, she is deeply committed to empowering the BIPOC community — a lifelong dream she is now living through her service. In addition, she serves on the Advisory Council, further strengthening her leadership role. Her unwavering passion continues to inspire positive change, supporting the growth and well-being of the youth she works with.
Meranda Wood Community Navigator Drawing from her own recovery journey and personal experiences, she brings a wealth of lived knowledge in overcoming homelessness, substance use disorder, and surviving human trafficking, as well as navigating the foster care system. Her passion for guiding young women from the streets to a productive, empowered lifestyle is truly inspiring. She is deeply committed to using her story to show others that transformation and hope are possible.

What we do

We provide direct and immediate connections to a wide range of community resources, with a strong focus on harm reduction, substance use education, and overdose prevention. Our advocates offer consistent guidance and mentorship, empowering our young & elders to make informed, healthy decisions and build a strong foundation for their future."
